Step Down Service

Our Step-down mental health care provides essential support for individuals transitioning from inpatient psychiatric services back to community living, focusing on rehabilitation and independence.

The overall aim of our service is to bridge the gap between being in a hospital setting and transitioning to independent living.

We collaborate with patient flow & bed management teams to offer a gradual and structured pathway for individuals, ensuring they receive the right care, attention, and guidance at each step of their recovery process.

Home Care Service

Our Home Care Service provides professional support for individuals who wish to remain living safely and comfortably in their own home. We deliver personalised homecare that promotes dignity, wellbeing and independence, ensuring individuals receive the right level of care within familiar surroundings.

The overall aim of our service is to support people to live well at home, reducing risk, maintaining stability and preventing unnecessary hospital admissions.

We work alongside families and community professionals to provide dependable, compassionate care, offering reassurance and structured support at every stage.

How We Work

We provide structured care and support across the West Midlands, both within our Step Down Houses and in individuals’ own homes. Through collaboration with healthcare professionals, families and community services, we create clear, person-led pathways that help individuals feel positive about their future, their choices and their goals.

Our step down services are built around your hopes, goals and recovery journey. We provide safe, respectful and collaborative care that empowers individuals to manage their mental health, build life skills and actively participate in their communities, whilst ensuring support is always shaped around your individual needs and long-term aspirations.

What We Do

We work in partnership with referral agencies throughout the county and beyond. We support people who are in recovery, living with an ongoing mental health problem,  or learning disability. Partnership working is of importance, and we will work constructively with health and social work professionals to ensure that we offer a focused and robust service to our clients, in a caring, safe environment.

We welcome referrals from partners in health or social services.
